Taking the first step toward therapy is meaningful, and I'm glad you're here. These forms help me understand your history and what brings you in, so our first session can be spent on you rather than paperwork.

Please complete all four before your first appointment. You can fill them out on your phone, tablet, or computer — there's nothing to print, scan, or mail back.

How this works

  1. Choose a form below. Each one opens in a secure, encrypted page.
  2. For the three forms that need your signature, you'll enter your email address first and receive a private link. This extra step is what keeps your signature legally valid and your information protected.
  3. Fill it out and submit. Your answers come directly to me — nothing is stored on this website.
  4. That's it. No printing, no scanning, no email attachments.

Your contact details, date of birth, and insurance information. You can upload photos of the front and back of your insurance card straight from your phone.

Adult Self‑Report

The longest of the four, and the most helpful. It covers what brings you in, your history, and how things have been affecting your daily life. Take your time with this one — somewhere quiet, when you're not rushed.

Psychotherapy Practices Agreement

My policies on scheduling, cancellations, fees, payment, privacy, and how we'll communicate between sessions. Worth reading properly — it explains how the practice runs.

Agreement to Telehealth

Your consent for us to meet by video when circumstances make it hard to be in the room together. It covers your rights, confidentiality, and the limits of meeting online.

Private and encrypted

Every form is HIPAA‑compliant and encrypted end to end. Your answers go directly to me and are never stored on this website.

Go at your own pace

You don't have to finish everything in one sitting. If a question doesn't apply to you, it's fine to leave it blank and we can talk it through together.

Nothing here is a test

Some questions are personal. Share what you're comfortable sharing — there's no wrong answer, and everything is a starting point, not a verdict.
